I authorize Blaze Christian Fellowship, Inc to take photos of my child during VBS for purposes including, but not limited to, slide shows, end-of-week video, social media, or church website.

Medical Release

I authorize Blaze Christian Fellowship, Inc to seek medical care for my child should I be unreachable in the event of an emergency, accident or illness.

Release of Liability

VBS provides a variety of learning experiences for children that may include physical exercise. Although I understand that Blaze Christian Fellowship, Inc and their representatives will make reasonable effort to provide a safe environment, I am fully aware of the risks inherent in participating in VBS activities and agree to hold harmless Blaze Christians Fellowship, Inc from liability of injuries sustained by my child.
